Community Home Care
There’s no place like home—especially when you’re being rehabilitated after a serious illness or a medical procedure. Being in a familiar surrounding with friends and family encourages the healing process.
Community Home Care offers you professional medical care in the comfort and convenience of your home. Community Home Care’s skilled health care team is committed to your recovery. Your health care team will work closely with you and your physician to provide the skilled medical care and comfort you deserve during your recovery. We want you to Live Well and Be Well.
Community Home Care provides in-home skilled services to homebound patients and our services are covered by:
- Medicare.
- Medi-Cal.
- Worker’s compensation.
- Most private insurance companies.
To be eligible for Home Care:
- You must have physician orders.
- Be homebound.
- Require skilled care.
Specialists will manage your account to ensure the correct process is followed for insurance and personal-pay accounts.
How we can help you:
- Registered nurses can help you with wound care, medication management, tube feedings, pain management, diabetic education and catheter care.
- Home health aides can assist you with activities of daily living, such as personal care, bathing and dressing.
- Physical therapists can help you with transfer and gait training, evaluation of home safety and equipment needs, and therapeutic exercises to build strength, improve mobility and decrease pain.
- Occupational therapists can help you gain independence in self-care, increase coordination and improve the dexterity of your upper extremities. They can also evaluate adaptive equipment that you may need to reduce pain and increase mobility.
- Speech therapists can provide a comprehensive evaluation and treatment of speech, language, voice, swallowing and cognitive disorders.
- Medical social services staff can assess your social and emotional needs, provide counseling, and suggest financial and community resources.

Personal Care
Live Well. Be Well. That is the commitment of our personal care team. We understand that being independent, living with dignity and maintaining quality of life are three of the most important goals we can help you reach. Our personal care attendants provide anywhere from two-to-24-hour in-home services to meet your needs or the needs of your loved one—by the day, the week or the month. And you don’t need a medical order to get help as this is a private-pay service open to anyone.
There are many ways we can help you stay in your home and maintain as much independence as possible. We can help you with personal care and grooming, bathing, grocery shopping, meal preparation, light housekeeping, laundry, prescribed exercises or short walks, and even watching the children as you get some much-needed rest. We’re also available to help with transportation. Perhaps you have a doctor’s appointment, want a haircut or would like to go to church, we’ll take you there. Finally, your loved ones might need a break now and again. Let us help you give them the gift of rest.
We’re here to provide exceptional care for you in your home. We routinely assist people recovering from surgery or a serious illness. We help patients who have been diagnosed with debilitating diseases such as multiple sclerosis, muscular dystrophy, Parkinson’s or Lou Gehrig’s disease. We provide elder care to keep your family member living in a loving, comfortable and familiar environment. We also support parents who have children with special needs. Oxygen and Medical Equipment
Conveniently located just off Highway 49 at Stockton Street, Sonora Oxygen and Medical Supply is an oxygen and medical equipment service. They are open Monday through Friday, 9 a.m. to Noon and 1 - 5 p.m. (We're closed from lunch, Noon - 1 p.m.)
